PRIVACY NOTICE
To carry out my work as a professional practitioner in hypnotherapy, I hold some information about my clients and request information relevant to the service I offer to you, the client. I care about your privacy and am committed to protecting it. The following information is to state that I hold your data securely.

The type of personal information collected to give you the best service possible.
I collect and process the following information: name, address(es), phone number(s), email address(es), date of birth, registered doctor, relevant medical information (including but not limited to diagnosed or undiagnosed mental health conditions), medications (prescribed or otherwise) (diagnosed or non-diagnosed mental health conditions). I will also manually take written notes of some of your thoughts, emotions, goals, ambitions, and other personal information during consultations and any therapeutic session work.

How personal information is obtained and why it is obtained
Initial Form Submission
The details I request on the initial enquiry form will be kept on electronic file. If you decide not to go ahead or that I can not help you, your information will be securely destroyed within one calendar month after the last point of contact.

If we proceed further
Most of the personal information processed is provided directly by you for the following reasons: To help me give you, the client, the best possible service I can within my current abilities and to help you achieve the best results through my services. For ethical reasons and your safety, some of the information gathered may need to be referred to your registered doctor or other medical professional for your safety, including but not limited to any emergency reasons protecting your well-being. Under the UK General Data Protection Act (UK GDPR), the lawful bases I rely on for processing this information are under your consent. You can remove your consent at any time. You can do this by contacting me, Michael Blackledge, via my contact details aforementioned herein.
How your personal information is stored.
Other than anything not related to the professional therapeutic services offered, physical paper, electronic filing, video/and or audio recordings that may expose your identity will be kept for two years from the start to end dates of services offered, unless directed by you in writing, to be destroyed at an agreed earlier date, after which, all physical and electronic information of you will be disposed by complete electronic file wipe from any electronic storage system on all sources it has been saved on – either remotely on a cloud system or any other physical apparatus including, but not limited to compact disc, USB fob key, external or internal hard drive system, or any other electronic recording device. Any physical notes and other information will be shredded privately and securely incinerated.

Data breaches
If there is a data breach affecting the confidentiality of your data I will report this to the ICO and notify you within 72 hours of the breach.

Your right of access - You can ask for copies of your personal information.
Your right to rectification - You have the right to rectify personal information you think is inaccurate. You also have the right to complete information you think is incomplete.
Your right to erasure - You can ask for your personal information to be erased securely and thoroughly, provided this is in keeping with legal obligations.
Your right to restriction of processing - You have the right to ask for the processing of your personal information, provided this is in keeping with legal obligations.
Your right to object to processing - You have the right to object to processing your personal information, provided this is in keeping with legal obligations.
Your right to data portability - You have the right to ask that transference of the personal information you have given be transferred to another organisation or yourself, provided this is in keeping with legal obligations.
You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond.
